REGISTER WITH THE PRACTICE?

To register as a patient, providing you live within the practice area as identified on the practice area map, you need to supply your medical card and proof of residency. If you do not have a medical card please complete a new patient registration form, which is available from reception. We advise all new patients to make an appointment with the health care assistant for a new patient check, so we can obtain details of your medical history and current medication, as it can take time before we receive your records from your previous doctor.
